Cancellation Policy

LIABILITY WAIVER The student hereby represents that he/she is physically fit and able to receive and participate in the prescribed course of instruction. In consideration of being allowed, or my child being allowed to attend classes for a trial period on the dates above listed, I hereby agree to observe strictly, the rules of the school and I acknowledge that failure to observe these rules, adds to the possibility of myself or other members of the school being injured. I hereby release Merritts Ultimate Taekwon-Do; it’s Principals, Employees and Agents of and from any claim for loss, injury or damage to persons or property sustained in the course of any training program or function of the school, however caused, or while on the premises. I agree that failure to observe these rules may result in my expulsion.I assume all responsibility for any accidents or injuries that may occur as a result of my/his/her participation in the activities of the School. I understand, and agree that in the event of any illness or injury, medical attention in the manner of First Aid Treatment may be rendered, and I further agree that I/he/she may be taken for medical treatment in the event of any illness or injury at any time. I also agree that I will be responsible for any and all costs incurred in such an event. I have read and do fully understand all of the provisions contained herein. TERMS AND CONDITIONS Valuables Merritts Ultimate Taekwon-Do is not responsible for the loss or theft of or damage to any personal property. Do not bring any valuable or personal property to any Merritts Ultimate Taekwon-Do location. Assumption of Risk The participant acknowledges that the use of the facilities and equipment thereof has a certain inherent element of risk to the participant’s physical person and property. The participant hereby voluntarily assumes all risk of accident or damage to his or her person and property and loss thereof and hereby agrees that neither Merritts Ultimate Taekwon-Do nor any of its affiliates, officers, servants, agents, members, instructors or employees shall be liable for any claim, demand or liability of any kind for or on account of personal injury or damage or loss of any kind sustained by any person other than that caused by the willful actions or gross negligence of Merritts Ultimate Taekwon-Do or its employees.
